Patient Information and Office Policies

  • Phones are answered from 7:30am to 4:30pm, excluding Thursdays.

Phone messages—Will be returned if possible in between patients, if not in the morning, or
before leaving for the day.  Messages will be returned during regular business hours.


  • Medications will not be called out after hours. Pain medications will be refilled with appointment only.
  • SAMPLES—will be given at the time of appointment.
  • AFTER HOURS—No refills or new prescriptions will be called out routinely, May be given at doctor’s discretion ONLY UNDER CERTAIN CIRCUMSTANCES OR BY THE DOCTORS APPROVAL.

Doctor has established a relationship and trust with other physicians that he refers to and it is his opinion that they will give the best care available. If you cannot make the appointment for whatever reason, please call and cancel or reschedule with the specialist.


When your appointment is made, it is a fifteen (15) minute visit for health issue. If a family member is accompanying you and they need to be seen they will need to make an appointment; if there is time available, the doctor will see them. If he is too busy, another appointment will need to be made.  No walk-in appointments unless it is an urgent situation.

In order for the doctor to be on time with other patients that have scheduled appointments, please be prompt and on time for you appointment. If you are more than ten (10) minutes late to your scheduled appointment you will have to reschedule. There is a $25 charge for no showed appointments, this includes when you are late and over the allotted time give.


If you have issues, please let Dr. Wolff or Chris Wolff know. Don’t address the issue yourself, this creates animosity between staff members and patients. This is a public area, and proper respect is needed to be shown for the staff and other patients. We welcome your children, but the children need to be under your control and behave while in the office.


Payment is due at the time of service. It is your responsibility to inform the office if your insurance has changed. Your contract is between you and the insurance company—you must abide by the policy you have. The doctor also has an agreement with the insurance company and he will follow that agreement. Dr. Wolff has the right to refuse service for patients who have a balance that exceeds $300or nonpayment within 3 months.


It is the patient’s responsibility to inform staff of any changes. Please include all working phone numbers so that you can be reached.


An appointment is needed for all paperwork to be filled out. If the paperwork is brought in to be filled out, it will not be done the same day (depending on the paperwork it could talk up to one (1) week). Extra paperwork will have an extra fee. If paperwork is just dropped off there is a charge for this service.


All acute care need to be addressed through the emergency room first and Dr. Wolff will do the follow up care. Acute means things like motor vehicle accidents, chest pain, deep lacerations, broken bones, etc.

As of January 5, 2009 we are no longer accepting checks.

We take Visa, Master Card, and Discover. Sorry for any inconvenience this may cause.

Due to the number of no show and last minute cancellations, the following policy is now in effect:

A patient calls and cancels an appointment with less that 24 hour notice; there will be a $25.00 fee. If the patient cancels last minute and the appointment time can be utilized by another patient, than the patient will receive a missed appointment status. This is very important to keep track. Patients that have missed 3 appointments in a year will be dismissed from the practice. If a patient has missed two appointments within a 2 month period will also be dismissed.  It is very important that patients come to their scheduled appointments.

Work Comp Injuries

Rock Ridge Family Medicine does not see work comp injuries. If you are injured at work, you must contact your employer as soon as possible. The employer and the employer’s work comp insurance company are contracted with their own providers. Rock Ridge Family Medicine is not a contracting provider for any work comp company. Since you are a patient of Rock Ridge Family Medicine, this does not mean that Dr. Wolff will make an exception. Dr. Wolff will not treat any injuries resulting from work.

Outside Lab and Radiology Results 

Dr. Wolff, and fstaff realize that you are concerned with your health and getting results of tests is very important to you and the providers. Lab Tests may take up to 5 days to get back and imaging results can take up to 3-5 days. Please allow at least 3-5 days for Kelli to call you with the results.

Dental Care

Dr. Wolff and Micah VanNover, PA are unable to treat dental problems. Patients who need to see a dentist will be referred to Good Samaritan Clinic. At the clinic, the doctor will assess the problem and refer you to a dentist who they contract with.

“Red Flag” Policy

The “Red Flag” policy has been established by the federal government effective May 1, 2009. This policy is set forth to protect consumers against identity theft within offices that provide a payment or credit to consumers. Rock Ridge Family Medicine is in full compliance with this new policy, and has set new guidelines to comply with this government law. Rock Ridge Family Medicine staff has been instructed to scan and/or photocopy photo identification for ALL patients, new and existing. This is verified with the names on the insurance cards. If there is a verification problem the patient/client will be questioned and asked to verify their relationship to the patient being seen. If we are unable to obtain identification verification, insurance will not be billed and a cash payment will be required.

Rock Ridge Family Medicine will try to provide the needed documentation and information to ensure the protection and safety of our patient’s identity.

Effective July 1, 2011

Rock Ridge Family Medicine will not file MVA claims to insurance companies.

All MVA office visits will be on a cash basis only. We will give you a receipt of your payments and you can request the office notes to submit a claim yourself.

1st Visit–$200.00 plus any OMT, X-Rays, Lab
2nd to 20th–$120.00 plus any OMT, X-Rays, Lab
21st—Pending Doctors approval
